Welcome to Dominion Surgical Associates. Weight loss surgery has helped millions of people shed excess weight and live healthier lives. It is a common treatment option for people with extreme obesity because it also involves training the person to transform his/her eating habits to maintain their ideal weight and thus live a healthier life.
To schedule an appointment, please call Monday through Friday from 8:30am to 5pm. If you find it necessary to change your appointment, please call our office at least 24 hours prior to your scheduled appointment time. If the physician has a medical emergency that requires we reschedule your appointment, please forgive us for any inconvenience it may cause you.
Your first visit
Please arrive to our office at least thirty minutes prior to your SCHEDULED appointment time. This allows you time to park and walk to our building (North Building, Suite 406). Parking can sometimes delay you, but you may use the valet parking in front of the Main Building. This will also give us time to enter your information and get you registered. If you are not at your appointment thirty minutes prior, we will have to reschedule you. As a new patient, you are requested to bring the following to your initial appointment:
•Completion of New Patient forms which are located on the Bon Secours Medical Group website. Please click here to access the forms.
•Insurance cards (no copies allowed)
•Your co-pay (if applicable). Self-pay patients, please have your payment ready at your first appointment.
•Referral from your PCP (it is the responsibility of each individual patient to obtain referrals. Our fax number is 804-285-4168 should you wish to have them faxed from your PCP’s office.)
•If other reports or films have been generated prior to your consult, please have these items available, i.e. mammogram films. These items will help facilitate your appointment with our providers.
Thank you in advance for your cooperation regarding the above information. If some of the items are not available at the time of you appointment, you may experience a delay in your appointment. If you have any questions about your visit please feel free to call the office so we can clarify any information ahead of time.
Our physicians acknowledge that providing specialized surgical care is dependent upon maintaining a collaborative relationship with referring physicians. We value our relationship with referring physicians and strive to keep them informed of your diagnosis and treatment, as well as your process while under our care. It is our policy to refer patients back to their primary care physician, so that he or she may oversee your routine medical care.
Patient Education/ Bariatric Surgery
All patients are required to attend an information lecture hosted by one of our surgeon’s office. During this time you will have the opportunity to meet the surgeons and their staff and have your questions answered about weight-loss surgery. For your convenience, the meetings are held throughout the day and evening. The meetings generally last for two hours. There is no cost for this lecture. You may bring family members or your support person; however, please no children.

•Causes, co-morbidities and criteria for surgery
•Possible risks and complications
•Summary/Questions and Answers
Choosing weight-loss surgery is a major life-altering decision that requires your initiative and commitment. The Informational Lecture is the first step in this process and gives you an opportunity to find out if weight loss surgery is right for you. You are under no obligation to move forward with surgery. It is up to you to schedule an individual consultation with a surgeon to move to the next step.
Please do not hesitate to call us Monday through Friday from 8:30am to 4:45pm, with any questions regarding your care. One of our nurses will discuss your concerns with you, and if necessary your physician will review your medical record. The nurse will then return your telephone call with specific directions and recommendations. Please remember to provide a telephone number where you may be reached during working hours.
For emergencies, our physicians are available to your 24 hours a day. You can call our main office number, 804-285-4133, and after hours you will be transferred to our on-call physician service. If at any time you are experiencing a life-threatening emergency, call 911.
Your records are confidential and will not be released without signed consent from you.
Our physicians maintain active privileges at St. Mary’s Hospital. When non-emergency hospitalization is required, our staff will make the necessary arrangements for your admission. During your hospital stay, your physician will visit you on a daily basis to access your condition, make changes in your medical regimen and order any testing that is necessary. Prior to your discharge, you will receive specific instructions on your follow-up care. Please follow all instructions carefully. Insurance and payment information
We accept all insurance plans, including Medicare. Many plans require that you make a co-payment at the time of your visit. For self-pay patients, payment is expected in full at the time of service. To make it as easy as possible for you, we accept cash, checks, Visa, and MasterCard. Patients may contact the practice manager to discuss options for financial assistance.
