What to Expect During and After Your Visit
What to bring to your appointment:
- Completed Registration Packet
- ID & Insurance Card & Your Prescription card
- Insurance Referrals/Authorization if you have one
- List of your current medications
- ANY MRI, CT, Scan related to your diagnosis. Bring Disc and printed reports
- Please fax your papers to 877-817-1865 prior to your appointment if possible
Please make sure that your referring physician has completed insurance referral BEFORE your visit. If you are not sure if your plan requires a referral contact your insurance provider.
After your appointment, any internal referrals to see another Bon Secours provider will be completed within 7 days
Copay & Billing
Copay is due at the time of service. And any co-insurance that you may have. Acceptable methods of payment include-Mastercard, Visa, Discover or American Express, cash and check.
Any billing questions should be directed to our office at 220-4263.
Please arrive 15 minutes prior to your scheduled time so that we can make sure you have everything you need for the visit.
We try very hard to remain on schedule but emergencies do arise on occasion. You would be informed if that happens.
Imaging and Lab Results
Please bring your Imaging CD with you. Failure to do so could result in your appointment being rescheduled.
If we order images or labs, these results will post to your patient portal MyChart within 7 business days.
If you are not signed up for MyChart we encourage you do so and can provide you information on how to do this.
