As part of the overall closure of OLBH, all Bellefonte Physician Service practices (those owned by the health system) have also closed. A majority of the physicians and providers are staying in the local area. The most recent information regarding closing dates and, when known, new provider locations can be found by clicking here. To obtain copies of medical records please call 866-625-7130, fax 678- 710-7032 or email 60181_Our_Lady_Bellefonte_Hospital@cioxhealth.com.
To have your medical records sent to a provider of your choice, a record release form will need to be completed. Your medical records will be available to you for several years beyond OLBH’s closure.
