To request a copy of your medical records, download the Release of Information form (see below) and fax or mail it to the appropriate facility. Please be advised there may be a cost involved for this service.

To have your medical records sent to a provider of your choice, a record release form will need to be completed. Your medical records will be available to you for several years beyond OLBH’s closure.

Medical records will not be released without a written authorization. To authorize us to forward a copy of your medical record directly to a physician, you must complete the Authorization to Release Protected Health Information form, which is available from our offices.
