physician order process

The Diabetes Treatment Center requires a physician order for a patient to be seen. The physician can order education services via Connect Care or Fax.

Connect Care:  

 Choose any of the procedure codes below to request education services.

- Ref to Diabetes ED (REF20)

- Ref to Diabetes TX CTR (REF115)  

In the ‘Comments’ field, indicate any specific needs and if you are requesting Individual Education or Group Education.

-  Group Education is for patients with type 2 diabetes. Patients enrolled in a group class will receive the educational components recommended by the American Diabetes Association for Diabetes Self-Management Training.

-  Individual Education is ideal for patients with type 1 diabetes, gestational diabetes, pre-diabetes or individuals with learning barriers.  It is also available to those in need of education for insulin administration, insulin pump training, medication management and weight management.

Note:  Medicare requires its beneficiaries to be seen in a group setting unless they have a documented learning barrier or have received initial training previously and now need follow-up training.  Learning barriers include sight impairment, hearing impairment, speech/language barrier, mental/emotional challenges and physical limitations.  Please indicate one of these barriers in the ‘Comments’ field if individual education is warranted. 

Fax Form:  Fax the completed form to the preferred Diabetes Treatment Center location.  For your convenience, contact us to request an order form.
