Patients are seen by appointment. In most circumstances, we can accommodate same day appointments.

In order to provide you with the best possible care, we need the most up-to-date information about you. Please remember to bring your insurance card and a list of all medications you are taking.

Insurance and Payment Information
We accept most insurance plans including Medicare. Many plans require that you make a co-payment at the time of your visit.

For self-pay patients, payment is expected in full at the time of service. We accept cash, checks, VISA and MasterCard.

	Dr. Navleen Kaur Dr. Kaur received her medical degree from the Autonomous University of Guadalajara. She completed her residency at Mt. Sinai Hospital in Chicago. She is Board Certified in Internal Medicine. Dr. Kaur is fluent in Spanish, Persian, and Indian.
	Dr. Robert Rolfes Dr. Rolfes received his medical degree from the University of Cincinnati College of Medicine and completed his residency at Dwight David Eisenhower Army Medical Center at Fort Gordon, GA. He is Board Certified in Internal Medicine.
	Dr. David Kelly Dr. Kelly received his medical degree from the Medical College of Virginia and completed his residency at the University of Alabama in Birmingham. He is Board Certified in Internal Medicine.
	Dr. Jason Lee Dr. Lee received his medical degree from the Medical College of Virginia where he also completed his residency. He is Board Certified in Internal Medicine.
	Dr. Ronald Artz Dr. Artz received his medical degree from Northwestern University McGaw Medical Center in Chicago where he also completed his residency. He is Board Certified in Internal Medicine and Infectious Diseases.
	Dr. Brian Foster Dr. Foster received his medical degree from the University of Iowa College of Medicine in Iowa City, Iowa. He completed his residency in internal medicine at the Medical College of Virginia. He is Board Certified in Internal Medicine.
