To schedule an appointment, please call us Monday through Friday from 8:30 a.m. to 5 p.m.

If you find it necessary to change your appointment, please call our office at least 24 hours prior to your scheduled appointment time. If a medical emergency requires that we reschedule your appointment, please forgive us for any inconvenience it may cause you.

  • Your First Visit

    Please arrive at our office at least 30 minutes prior to your scheduled appointment time. This will allow you time to park and walk to our office in the North Medical Office Building. You may use valet parking.

    As a new patient, you are requested to bring the following to your initial appointment:

    • Insurance card
    • Referral form, if required
    • Copies of pertinent diagnostic tests and medical records including operative or pathology reports and/or X-rays
    • All current medications
    • Completed history and physical form, if available
    • List of allergies to medications and/or dyes
    • If you have been hospitalized or have had laboratory tests or x-rays for your current surgical problem at another facility, please have copies of the reports sent to our office prior to your first visit.

    Thank you in advance for your cooperation regarding the above information. If you have any questions about your visit, please feel free to call the office so we can clarify any information ahead of time.

  • Forms

    The following is a list of forms you will need for your first appointment. For your convenience, you may fill these out at home and bring them with you. But if you do not want to do that or you forget them, we have the forms at our office.

    mychart offers Bon Secours patients free personalized online access to portions of their medical records. It enables you to securely use the Internet to manage and receive information about your health online.
    With mychart you can:

    • Send a message to your doctor
    • Request an appointment
    • View your health summary
    • View lab results
    • Request prescription refills
    You can sign up for your mychart account at Bon Secours General Surgery or visit mybonsecours.com to receive your personal access code.
  • Referrals

    Our physicians acknowledge that providing specialized surgical care is dependent upon maintaining a collaborative relationship with referring physicians. We value our relationship with referring physicians and strive to keep them informed of your diagnosis and treatment, as well as your process while under our care. It is our policy to refer patients back to their primary care physician, so that he or she may oversee your routine medical care.

  • Insurance and payment information

    We accept all insurance plans, including Medicare. Many plans require that you make a co-payment at the time of your visit. For self-pay patients, payment is expected in full at the time of service. To make it as easy as possible for you, we accept cash, checks, Visa and MasterCard.
