Bon Secours Health System is to bring compassion to healthcare and to be good help to those in need, especially those who are poor and dying. As a system of caregivers we commit ourselves to help bring people and communities to health and wholeness as part of the healing ministry of Jesus Christ and the Catholic Church.

Welcome to Village Medical Associates. It is our sincere desire to give you the best healthcare possible. We specialize in compassionate family medicine and provide primary care throughout your family’s life.

Office Hours & Appointments
Our office is open Monday through Friday, 8:30 a.m. to 5:00 p.m. Patients are seen by appointment. The office is closed for lunch from 12:30 p.m. until 1:30 p.m. during which time the phones are transferred to our answering service. Please contact us 24 hours in advance if you need to cancel your appointment. Failure to cancel your appointment may result in a fee.

In order to provide you with the best possible care, we need the most up-to-date information about you. Please remember to bring your insurance card and a list of all medications you are taking.

Insurance and Payment Information
We accept most insurance plans, including Medicare. Many plans require that you make a co-payment at the time of your visit. For self-pay patients, payment is expected in full at the time of service. To make payment as easy as possible for you, we accept cash, checks, VISA and MasterCard. Patients may contact the practice manager to discuss options for financial assistance. If you have any questions regarding your bill, please contact our business office at (804) 521-9300 or (800) 732-7072.

Bon Secours ConnectCare (Summer 2009)
Village Medical Associates is pleased to announce the launch of Bon Secours ConnectCare. By establishing this electronic medical record (EMR) system, we are converting your current paper medical chart to a secure, electronic medical record.

As a patient, you will benefit in many ways, including increased patient safety, an enhanced physician-patient relationship and better physician-to-physician communication regarding your healthcare.

ConnectCare will also enable physicians to order procedures and prescriptions, have more efficient access to results and make referrals to specialists – all through this protected network. We look forward to bringing the future of healthcare to you.

Physicians
Ed Glynn, M.D.
Dr. Glynn received his medical degree from Eastern Virginia Medical School in Norfolk and completed his residency at Saints Family Medicine Residency in Oklahoma City. He is board certified in Family Practice. Dr. Glynn also serves as the Medical Director for Bon Secours ConnectCare.
Ahsan Siddiqui, M.D.
Dr. Siddiqui received his medical degree from Ross University, School of Medicine and completed his residency with VCU-Riverside Family Medicine Residency in Newport News, Virginia. He is a member of the American Medical Association and American Academy of Family Physicians. Dr. Sidduqui is Board Certified in Family Medicine.
Monica Forth, M.D.
Dr. Forth received her medical degree from Howard University College of Medicine, and she completed her residency at University of Oklahoma, where she served as Chief Resident. She is board certified with the American Board of Family Physicians. Dr. Forth enjoys practicing the full spectrum of family medicine and has a special interest in Women's Health and Pediatric care.

Telephone Calls
Have a question? That’s why we are here! Feel free to call us Monday through Friday, 7:00 a.m. to 7:00 p.m. Our staff will make every effort to answer your call. If all of our staff are assisting patients and you receive voicemail, please leave your name, date, two telephone numbers where you may be reached and a brief description of your medical problem so that we may process your call promptly.

Since our physicians are caring for patients, only in the event of an urgent medical situation will the physician be called to the telephone immediately.

Emergencies
For emergencies, our physicians are available to you 24 hours a day. You can call our main office number (804) 794-5880, and after hours you will be transferred to our on-call physician service. If at any time you are experiencing a life-threatening emergency, call 911.

Referrals
If your physician has referred you to a specialist, we request a 48-hour notice for processing referrals, except for emergencies.

Prescription Refills
For your convenience, refill requests for medications other than controlled drugs may be requested through your pharmacy, and your pharmacy will submit the request to us. Medication refill requests should be made during normal business hours. We require 48 hours to process refill requests. We are not able to provide medication requests after hours.

Please remember that in certain situations, appropriate medical care requires a patient examination before a prescription can be filled.

Medical Records
Your records are confidential and will not be released without a signed consent from you.
