The Mission of the
Bon Secours Health System is to bring compassion to healthcare and to be good help to those in need, especially those who are poor and dying. As a system of caregivers we commit ourselves to help bring people and communities to health and wholeness as part of the healing ministry of Jesus Christ and the Catholic Church.

Welcome to Patterson Avenue Family Practice. It is our sincere desire to give you the best health care possible. We specialize in family medicine and provide compassionate primary care through all stages of life.

Appointments
Our office is open Monday through Friday from 8:00 a.m.–5:00 p.m. Patients are seen by appointment. In most circumstances, we can accommodate same-day appointments upon your phone call. Please contact us if you need to cancel your appointment. Failure to cancel your appointment in 24 hours in advance may result in a fee.

In order to provide you with the best possible care, we need the most up-to-date information about you. Please remember to bring your insurance card and all medications you are taking.

Insurance and Payment Information
We accept most insurance plans including Medicare. Many plans require that you make a co-payment at the time of your visit. To make it as easy as possible for you, we accept cash, checks, VISA, and MasterCard. If you have any questions regarding your bill, please contact our business office at (804) 521-9300 or (800) 732-7072.

  • Chronic care for diseases such as diabetes, hypertension, and obesity

  • Acute minor injuries and lacerations
  • Women's services, including pap smears
  • Minor surgery and lesion removal
  • Sports physicals and back-to-school physicals
  • Well child care and immunizations
  • On site x-ray and laboratory

Physicians
Michael Taylor, M.D.
Dr. Taylor received his medical degree from Virginia Commonwealth University School of Medicine and completed his residency at Kanawha Valley Family Practice Center in Charleston, W.VA. He is Board Certified in Family Practice.
Barry Wein, M.D.
Dr. Wein received his medical degree from Thomas Jefferson Medical College in Philadelphia and completed his residency at Riverside Hospital in Newport News. He is Board Certified in Family Practice
Mark Barr, M.D.
Dr. Barr received his medical degree from Virginia Commonwealth University School of Medicine and completed his residency at Jackson Memorial Hospital in Miami. He is Board Certified in Family Practice.
Tami Fountain-Ellis, M.D.
Dr. Fountain-Ellis received her medical degree from Virginia Commonwealth University School of Medicine and completed her residency with Riverside Hospital in Newport News. She is Board Certified in Family Practice.
Matthew Payne, M.D.
Dr. Payne received his medical degree from Virginia Commonwealth University School of Medicine and completed his residency with Chesterfield Family Practice Residency Program. He is Board Certified in Family Practice.
Amy Teague, PA-C
Ms. Teague attended the University of Virginia in Charlottesville for her undergraduate degree. She received her Physician Assistant degree from Emory University School of Medicine in Atlanta.
Carolyn Moncure, A.N.P.
Ms. Moncure attended Rhode Island College in Providence, R.I., for her undergraduate degree. She received her nurse practitioner degree from Virginia Commonwealth University School of Medicine.
Haroon Hyder, M.D.
Dr. Hyder received his medical degree from the University of Mysone in Mysone, India and completed his residency at Carraway Methodist Medical Center in Birmingham, Alabama. He is Board Certified in Family Medicine.



Soeria Sanderford, F.N.P.
Ms. Sanderford attended The Red Cross School of Nursing in Tehran. She received her Masters of Science in Medical Surgical Nursing from The Catholic University of America and her Family Nurse Practitioner degree from Virginia Commonwealth University. Ms. Sanderford served as a captain in the United States Army at Fort Lee, Virginia. She is fully licensed by the State of Virginia Board of Nursing.

Thomas Hill, M.D.
Dr. Hill attended Berea College located in Berea, Kentucky for his undergraduate degree. He received his medical degree from the University of Louisville, School of Medicine and completed his family medicine residency from the Eastern Virginia Medical School. Dr. Hill is board certified in Family Medicine.

Telephone Calls
Have a question? That’s why we are here! Feel free to call us Monday through Friday, 7:00 a.m. – 7:00 p.m. Our staff will make every effort to answer your call. If all of our staff are assisting other patients and you receive voicemail, please leave you name, date of birth, two telephone numbers where you may be reached and a brief description of your medical problem so that we may process your call promptly. Since our physicians are caring for patients, only in the event of an urgent medical situation will the physician be called to the telephone immediately.

Emergencies
For emergencies, our physicians are available to you 24 hours a day. You can call our main office number (804) 741-6200, and after hours you will be transferred to our on-call physician services. If at any time you are experiencing a life-threatening emergency, call 911.

Referrals
If your physician has referred you to a specialist, we request 72-hour notice for processing referrals, except for emergencies.

Prescription Refills
For your convenience, refill requests for medications other than controlled drugs may be requested through your pharmacy, and your pharmacy will submit the request to us. Medication refill requests should be made during normal business hours. We request 48 hours to process refill requests. We are not able to provide medication requests after hours.

Please remember that in certain situations, good medical care requires a patient examination before a prescription can be filled.

Your records are confidential and will not be released without a signed consent from you.
